Key Responsibilities
- Provide practical and emotional support to individuals, assisting with daily living tasks such as personal care (washing, dressing), meal preparation, and household management.
- Administer medication according to training and care plans, accurately documenting all actions.
- Support individuals in accessing community services, engaging in social activities, and pursuing hobbies and interests.
- Assist with mobility and transfers, using appropriate equipment where necessary, ensuring safety and comfort.
- Maintain accurate and up-to-date records, including daily notes, incident reports, and care plan reviews.
- Communicate effectively with service users, their families, and other healthcare professionals, reporting any concerns promptly.
- Promote independence, choice, and dignity, respecting the individuality and privacy of each person.
- Implement tailored care plans, ensuring all support is person-centred and reflective of individual needs and preferences.
