Key Responsibilities
- Conduct comprehensive assessments to identify patient needs, including medical, psychosocial, and environmental factors.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and healthcare providers.
- Coordinate and facilitate access to necessary healthcare services, community resources, and support programs.
- Educate patients and families on their health conditions, treatment options, and self-management strategies.
- Advocate for patients' rights and ensure continuity of care across various healthcare settings.
- Maintain accurate and timely documentation of all case management activities and patient interactions.
- Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.
- Evaluate the effectiveness of care plans and make modifications to optimize patient outcomes.
- Ensure compliance with all relevant federal, state, and local regulations and ethical standards.
- Manage a caseload effectively, prioritizing tasks and maintaining a high level of organization in a remote setting.
Required Skills
- Bachelor's degree in Nursing, Social Work, or a related healthcare field (Master's preferred for Social Work)
- Current, unrestricted licensure as a Registered Nurse (RN) or Licensed Master Social Worker (LMSW) in Michigan
- Minimum of 2 years of experience in healthcare case management, care coordination, or a similar role.
- Proficiency with electronic health records (EHR) systems and standard office software.
- Excellent communication, interpersonal, and negotiation skills.
- Strong organizational and time management abilities, with a capacity for independent work.
- Demonstrated ability to exercise critical thinking and problem-solving in complex situations.

Preferred Qualifications
- Certified Case Manager (CCM) certification.
