Job Description
We are seeking a highly skilled and detail-oriented Medical Coder specializing in Inpatient Records to join our remote team. As a vital member of Career.zycto’s network of professionals, you will play a crucial role in ensuring the accuracy, completeness, and compliance of medical records for inpatient services. In this role, you will be responsible for applying appropriate coding guidelines (ICD-10-CM, ICD-10-PCS, CPT, HCPCS) to complex inpatient medical records, abstracting relevant information, and assigning correct codes to support accurate billing and data analysis. You will collaborate closely with clinical documentation improvement (CDI) specialists and medical staff, querying physicians for clarification when documentation is unclear or incomplete. A strong understanding of anatomy, physiology, medical terminology, disease processes, and pharmacology is essential to succeed in this position. Key Responsibilities
- Assign accurate ICD-10-CM, ICD-10-PCS, and CPT/HCPCS codes to inpatient medical records based on official coding guidelines and conventions.
- Abstract pertinent information from patient records, including diagnoses, procedures, and discharge summaries.
- Perform retrospective and concurrent reviews of medical documentation to ensure coding accuracy and compliance with regulatory requirements.
- Identify and resolve coding discrepancies, incomplete documentation, or ambiguous information by issuing professional and compliant physician queries.
- Stay current with changes in coding guidelines, conventions, regulations, and industry best practices.
- Collaborate with Clinical Documentation Improvement (CDI) specialists, physicians, and other healthcare professionals to ensure complete and accurate documentation.
- Participate in quality assurance and auditing processes, providing feedback and contributing to process improvements.
- Maintain high productivity and quality standards while working independently in a remote setting.
- Protect patient privacy and ensure the confidentiality of all medical information in accordance with HIPAA regulations.
Required Skills
- Minimum of 3 years of recent inpatient coding experience in an acute care setting.
- AHA or AHIMA coding certification (e.g., CCS, RHIA, RHIT).
- Expert knowledge of ICD-10-CM and ICD-10-PCS coding systems.
- Proficiency with Encoder software and Electronic Health Record (EHR) systems.
- Strong understanding of medical terminology, anatomy, physiology, and disease processes.
- Excellent analytical and problem-solving skills with a keen attention to detail.
- Ability to work independently, manage time effectively, and meet productivity targets in a remote environment.
- Exceptional written and verbal communication skills for effective physician querying and team collaboration.
Preferred Qualifications
- Bachelor's degree in Health Information Management or related field.
- Experience with multiple EHR systems.
