Job Description

Career.zycto is seeking a dedicated and compassionate Full-Time Nurse Practitioner with a specialization in Geriatrics to join our dynamic healthcare team in East Norwalk, Norwalk. This vital role involves providing comprehensive primary care and specialized geriatric services to our senior patients, both in our clinic and potentially through limited home visits or skilled nursing facility rounds as needed. As a Geriatric Nurse Practitioner, you will play a pivotal role in managing complex chronic conditions, conducting thorough health assessments, developing personalized care plans, and advocating for the unique needs of the elderly population. Key Responsibilities

  • Conduct comprehensive geriatric assessments, including physical, cognitive, functional, and psychosocial evaluations.
  • Diagnose and manage acute and chronic health conditions common in the elderly, developing evidence-based treatment plans.
  • Prescribe medications and other therapies in accordance with state regulations and professional guidelines.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients' health problems.
  • Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated and integrated care.
  • Provide patient and family education on disease management, health promotion, and preventive care strategies specific to geriatrics.
  • Perform procedures within the scope of practice, as necessary, and assist with care transitions.
  • Maintain accurate, confidential, and up-to-date patient records.
  • Participate in team meetings, quality improvement initiatives, and continuing education pertinent to geriatric care.

Required Skills

  • Current Connecticut Advanced Practice Registered Nurse (APRN) license.
  • Board Certification as a Geriatric Nurse Practitioner or Adult-Gerontology Nurse Practitioner (AGNP).
  • Minimum of 3 years of clinical experience as an NP, preferably in geriatrics or adult primary care.
  • Strong diagnostic, assessment, and critical thinking skills.
  • Excellent communication and interpersonal abilities, with a patient-centered approach.
  • Proficiency in electronic health records (EHR) systems.
  • Ability to work autonomously and as part of a collaborative team.
  • Demonstrated empathy, patience, and cultural sensitivity towards older adults.

Preferred Qualifications

  • Master's or Doctoral degree in Nursing from an accredited program.
  • Experience in diverse geriatric care settings (e.g., clinic, skilled nursing facility, home health).
  • Bilingual proficiency (e.g., Spanish) to serve a diverse patient population.
  • Active DEA registration.
